Refer Someone to Nexus for NDIS Services
Know someone who could benefit from extra support in their daily life?At Nexus, we support people with disability to build independence, develop life skills, and stay connected with their community. Our services are person-centred and tailored to each individual’s goals under the NDIS.We welcome referrals from families, carers, support coordinators, local area coordinators (LACs), health professionals, educators, and community organisations.Whether you’re referring a young person transitioning from school, or an adult looking to increase independence and confidence, we’re here to help.
Who can you refer?
You can refer someone who:
- Is living with a disability, injury, or mental health condition
- Is an NDIS participant, or may be eligible for the NDIS
- Is preparing to leave school or has recently transitioned from school
- Would benefit from support to build independence, daily living skills, or social connections
- Needs assistance to engage in their community or achieve personal goals
How can we help
Our supports are flexible and tailored to each person’s NDIS plan and goals. We may provide:
- Assistance with daily living skills (e.g. cooking, budgeting, routines)
- Support to build independence and confidence
- Community participation and social connection
- Capacity building and skill development
- Support with routines, structure, and goal setting
- Group programs and peer-based activities
- Support coordination and advocacy (where applicable)
- Transition support (e.g. school to adult services)
What happens after you refer someone?
- We’ll contact the person (or their parent/guardian) to confirm their interest
- We’ll arrange a time to understand their goals, strengths, and support needs
- If suitable, we’ll develop a personalised support plan aligned with their NDIS goals
